| Models Affected: DMV250 and DMV400 fitted to CUTLER-HAMMER
and EATON LV switchboards manufactured from May 2001 to 16th
March 2009. Identification: LV Distribution boards type QCC, DCC, ECHIDNA,
EWQ, EWD, and “Plus” variants, that are fitted with a 250A or 400A
main isolator branded HOLEC or EATON with a clear cover and a black
lever-type rotary handle (as shown in the above photographs)
The Hazard: The handle may fail to operate as intended under normal
use. This could lead to an operator suffering an electrical shock
after removing the switchboard escutcheon plate, if normal safety
procedures are not followed to confirm the electrical supply has been
isolated.
